Identity Proofing
It is required to perform Identity Proofing of an individual before provisioning credential access. Specifically this is required before allocated a Multi-Factor Authentication (DUO) hardware/software token. The Helpdesk and other designated IT staff act as agents of the UW Oshkosh Local Registration Authority.
Identity Proofing Requirements
If a user needs to use MFA it is required to verify the identity and affiliation of the individual with UW Oshkosh. It is required to perform identity proofing in the following scenarios:
- Provisioning MFA access for a new user
- Reissuing MFA access to an existing user
- Provisioning MFA one-time pass codes.
Please contact the IT Help Desk for further instructions on how to complete the identity proofing process. If you are unable to be physically present for the procedure, it will be required to perform a remote session via webcam.
Identity requirements:
- Proof of identity - who the individual is
- Examples of proof of identity
- State issued photo ID
- Passport
- US Military Card
- Examples of proof of identity
- Proof of affiliation - individuals association with the University of Wisconsin Oshkosh
- Examples of proof of affiliation
- UW Oshkosh ID Card
- HR Verification form
- Examples of proof of affiliation
Verify identification information:
- Full Name matches on both forms of ID
- Photo matches individual
- Expiration Date (MUST NOT BE EXPIRED)
