This document summarizes what to do should you lock your account after answering your contingency questions incorrectly while trying to generate a temporary static one time password.
A UW Digital ID customer can get locked out of their contingency access if they incorrectly answered their contingency access questions three (3) times. If this is the case, you will see the following message when you try to request a temporary access code:
To unlock your contingency code, you will need to visit a Local Registration Authority (LRA) to get credentialed and unlock your OTP access. Please refer to this KB document for credentialing instructions and where to find your LRA: 33434
If contingency access is locked, you cannot request temporary codes, but your regular OTP token will continue to work.
Navigate to https://uwdigitalid.wisconsin.edu and log in with your institutional credentials.

When prompted to, enter your OTP code to finish logging in.
Search for the customer's request by entering the customer's name into the Name field and selecting the "Completed" status.
Select the customer's OTP request.
Credential the customer as described in this KB document: 68615
Click Unlock Contingency Access in the Available Actions panel.
Once you see User has been unlocked from contingency access, the customer can once again request temporary passwords from the UW Digital ID app.
