Completing the Application Type Page
What is the purpose of the Application Type page?
The Application Type section of the initial review smartform allows study teams to select the appropriate IRB application for a study or project from a list provided on the first page of this section. Study teams will see a different IRB application form based on the application type selected.
If you wish to request for the UW-Madison to cede IRB review to another IRB, these requests are handled through a different process (see here). Please contact the Health Sciences IRBs Office Reliance Team at irbreliance@medicine.wisc.edu if you have questions.
When does the Application Type section page display?
The Application Type page will display for every new IRB application. The Application Type page can be found in the General Study Information section of the initial review application smartform.
How do I complete the Application Type page?
Study teams complete the Application Type page by selecting the one application type that is appropriate for their study or project. A description of each application type is provided via the Help link on the far right hand side of the page. Selecting the appropriate application type is important so that the study team does not complete unnecessary parts of the form or provide insufficient information to the IRB.
What if I make changes to the application type while preparing the initial review smartform?
Changing the application type may have a significant impact on what parts of the IRB application display, so it is important to choose the correct application type before completing other parts of the application.
What if I am not sure about which application type to select?
More information about application types can be found in the Application Type Guidance. If you are not sure about which application type to select, please contact asktheirb@hsirb.wisc.edu for assistance. Please do not guess which application type should be selected as this may result in completing unnecessary parts of the application or in substantial revisions to the application during the pre-review process if the wrong application type is selected.
